'Very Integrated Program (VIP): Smoking and other lifestyles, co-morbidity and quality of life in patients undertaking treatment for alcohol and drug addiction in Sweden'
Most patients with alcohol and drug addiction have other risky lifestyles and non-communicable diseases (NCDs), adding to their morbidity and premature mortality.
Those are, however, potentially preventable. The aim was to identify and compare the patients in treatment for alcohol and drug addiction and identify important factors for high risk.

Risky consumption of alcohol is a global problem. More than 3.3 million deaths annually are associated with risky use of alcohol. People with high alcohol consumption often require planned and emergency
surgical procedures.
The objective is to assess the effects of perioperative alcohol cessation interventions on rates of postoperative complications and alcohol consumption.

Major interest in media the last few days, about Swedish doctors requiring patients to be alcohol-free before surgery. For patients with excessive alcohol consumption, it may halve the risk of complications during the operation. This includes the risk of infections, wound complications, intensive care and death.

We arranged Gold Standard Programme (GSP) courses on smoking and alcohol cessation intervention for staff in health care. The GSP course is related to the national initiative smoke free operation and the aim is to reduce surgical complications related to smoking and alcohol. After completing the course, participants can enroll patients in intensive smoking and alcohol cessation treatment. The courses are arranged in collaboration with Lund University, WHO-CC: Evidence-based Health Promotion in Hospitals & Health Services and Addiction Centre, Region Skåne.

World No Tobacco Day – Seminar

We arranged a seminar together with Kunskapscentrum levnadsvanor och sjukdomsprevention, on World No Tobacco Day, May 31, in Malmö.
The focus is "Tobacco and heart disease", and the aim of the seminar is to inform and disseminate knowledge about the link between tobacco use and cardiovascular disease, and the chance of reducing the risk by quitting the use of tobacco.

Publication of LIVE INCITE Request for Tenders

The LIVE INCITE project - funded by EU, coordinated by Stockholm County Council, and including Hospital Clinic Barcelona and Bispebjerg-Fredriksberg Copenhagen - challenges the market to develop a digital solution to support life-style changes related to surgeries. The aim is to reduce complications from surgeries related to risk factors such as smoking and risky drinking. Market stakeholders are invited to submit offers to participate in a pre-commercial procurement (PCP) process over three phases (solution design, prototype development, and original development and pilot). 2017 was a great year for our WHO-CC and we are so grateful for the strong support and collaboration we have received, to our work towards more effective treatments and better health gain for patients.
Worldwide, implementation is a major challenge in the clinical health care, therefore the focus in our WHO-CC is on the very new model for Fast Track Implementation including development of useful definitions and description of business cases. This focus will continue for the coming years.
A few highlights from 2017: Presentation of new evidence, Publication of 15 scientific articles + 1 book chapter, Implementation seminar on evidence-based teaching, New business case, among many other activities.
We are looking forward to next year and want to wish you all a Merry Christmas and a Happy New Year!

Self-reported needs for improving the supervision competence of PhD supervisors from the medical sciences in Denmark
Quality of supervision is a major predictor for successful PhD projects. The aim of this study was to explore PhD supervisors' self-reported needs and wishes regarding the content of a new program in supervision, with a special focus on the supervision of PhD students in medical fields.
Overall, the results indicated a general interest in improved competence and development of a new supervision programme. Those who were not interested argued that, due to their extensive experience with supervision, they had no need to participate in such a programme. The analysis revealed seven overall themes to be included in the course. The clinical context offers PhD supervisors additional challenges that include the following sub-themes: patient recruitment, writing the first article, agreements and scheduled appointments and two main groups of students, in addition to the main themes. In conclusion, the PhD supervisors reported the clear need and desire for a competence enhancement programme targeting the supervision of PhD students at the Faculty of Health and Medical Sciences. Supervision in the clinical context appeared to require additional competence.

Tobacco smoking is still one of the main risk factors for Swedish burden of diseases and it contributes largely to inequity in health. Each day someone dies from the consequences of tobacco use. Tobacco-free week is a national campaign working for a smoke-free Sweden. It is also a part of the national work "Tobacco Endgame - Smoke Free Sweden in 2025".

Yesterday’s seminar: Are we doing what’s best for the patients?

Great enthusiasm yesterday when 90 persons attended our seminar "New knowledge about education in patient-oriented health promotion - Are we doing what’s best for the patients?"
Staff and patients need more knowledge and evidence-based education in clinical health promotion! That’s one of the conclusions from yesterday’s seminar.
Representatives from Lund University, the health care sector, patient organizations, regional and national decision makers attended. Seven presentations covering different aspects and a panel discussion with six representatives.
Some of the interesting topics were: new plans for a Master of Clinical Health Promotion, a good example from the University Hospital in Malmö where they have an active lifestyle programme for patients before kidney transplant, and we need a complete curriculum for education in patient-oriented health promotion.
“We need to prioritize and work together - taking one thing at a time” was one of the final comments in the panel discussion.
Last conclusion: Are we doing what's best for the patients? – Not yet, but we are working on it!

Strong for Surgery-Strong for Life

Updated meta-analyses announced recently show that shorter life style changes starting before surgery have a positive effect for patients. Complications after operations can be reduced by up to 50% this way. Only intensive programs aiming at cessation of smoking and alcohol for 4-8 weeks before surgery have an effect on risk reduction at surgery.
- Quitting of smoking and risky drinking is part of good perioperative care that should be offered to all patients in need, says professor Hanne Tönnesen from Lund University.

FEMALE GENITAL MUTILATION - More countries need to be included in clinical health promotion

A total of about 150 000 girls and women, born in, or with both parents (or mother) from, one of the 29 countries listed by EIGE (European Institute for Gender Equality) are registered in Sweden. These 29 countries are listed as high risk for female genital mutilation by EU and UNICEF and therefore recommended to be included when estimating national risk.
"We would like the Swedish national estimation to have a broader perspective and include more countries than today", says Vanja Berggren, project coordinator. Since many years, our Nordic neighbouring countries include more listed countries. for example from the Middle East. That would have an important impact on the Clinical Health promoting work in Sweden.
Strong for Surgery-Strong for Life

Unhealthy living habits increase the risk of complications after surgery. It is well documented that preoperative smoking and drinking cessation programmes of 4-8 weeks duration, significantly reduce the increased risk of complications after surgery.
Strong for Surgery - Strong for Life is a project by Swedish Society of Medicine with the aim to increase safety for patients during surgery through intervention against unhealthy living habits, as well as reducing the risk of future illness by supporting the maintenance of a healthy lifestyle.
By integrating more effective programme for lifestyle intervention in the clinical setting, we can improve treatment results and promote faster recovery.

Empower patients to support lifestyle change

Supporting lifestyle changes, improving outcomes of care LIVE INCITE aims to empower patients and support lifestyle changes in order to improve outcomes of surgical procedures as well as cost effectiveness of healthcare providers. The EU funded project is a collaboration between Sweden, Spain and Denmark. LIVE INCITE will conduct a three-step precommercial procurement, where the majority of the EU funding will be allocated to the companies that participate in solving care challenges. The project is currently in its preparatory phase, which includes a market analysis. Companies can register their interest here: www.ted.europa.eu

Visit from South Africa on Capacity Building and Training

Recently the center had a visit from Limpopo University, South Africa. The group consisted of clinicians and researchers. During their visit apart from getting a presentation of WHO CC, they were particularly interested in women's health programs in Sweden. In the region where they are active, northern South Africa, maternal health issues heavily needs to be strengthened.
The visit was planned and funded together with South African Medical research Council and FORTE (Swedish Research Council for Health, Working Life and Welfare)

Patients with high genetic risk can reduce risk with life style changes

Many see genetic risk as an inevitable fate, but for cardiovascular disease it does not seem to be the case. A new study shows clearly that even those with high genetic risk can reduce risk with life style changes to the same level as for people with low genetic risk.
It is possible to change the course of genetic risk – that is the truth!

New PhD focuses on immigrants’ health

Vanja Berggren works her first week at the centre and will start a research study. Her focus is on life-style behaviour among immigrants in Sweden and the effects on their health. Vanja has earlier conducted research in African settings and taught in the Middle East as well as at Lund university.

New study about life style factors and surgery well on it’s way

A study lead by Professor Hanne Tönnesen is ready to be published shortly. The aim of the study is to evaluate if lifestyle factors are reported to national surgical quality registers. The second aim is to determine if pre-operative interventions and post-operative complications, related to preoperative life style factors and interventions, are registered.
New researcher at WHO CC writes about snuff and E-cigarettes

Senior Consultant David Chalom from the department of Orthopedics at Hässleholm-Kristianstad-Ystad is a new PhD at the centre. His main project is ”Snuff, E-cigarettes and other lifestyles related to orthopaedic outcomes".
New award-winning literature for students

Nurses will learn more about smoke-free and alcohol-free surgery in the new Swedish literature: Omvårdnad och kirurgi. Read chapter 6 written by Professor Hanne Tönnesen about tobacco and alcohol as risk factors in surgery.
Swedish University Hospital introduces pre-operative alcohol cessation

Akademiska sjukhuset, Uppsala University Hospital, will introduce alcohol cessation prior to surgery in Orthopaedics and hand surgery. The purpose is to avoid complications and faster recovery for patients. The project will start on the 1st of October with these two clinics; the aim is that all operations at the hospital will have pre-operative alcohol cessation in the future. Akademiska sjukhuset is the third hospital in Sweden introducing such a programme for patients. Skåne University Hospital in Malmö and Visby hospital are already up and running with similar programmes. The hospital will also actively ask patients about alcohol consumption and offer help to lower consumption for patients at risk.
